Nursing Performance with Patients on Mechanical Ventilation - Systematized Literature Review

Patricia Moura Motta, Isabel Cruz

Abstract

In high complexity care, most critically ill patients require invasive ventilatory support, with the nursing professional having a fundamental role in the evaluation of these patients and in the functioning of the ventilator. In recent years, Nursing has been improving its knowledge and proposing improvements in its care practice, developing its own work methodology
Objective: to identify in the literature what is already written about nursing interventions for critically ill patients, particularly with mechanical ventilation Methods: integrative literature review, in which the PICOT strategy was used, in which P (patient) impaired spontaneous ventilation; I (intervention) performance of nursing professionals; C (control) does not apply; The (result) interventions to be applied by nursing professionals; T (time) seven days. 10 articles were used. Results: It is understood that monitoring patients on ventilatory support is a very important factor. From the integration of data collected from the functional tests, with continuous clinical observation, it is possible to act accordingly, preventing lethal complications and increasing the chances of success in the treatment of these patients. The nursing professional, when monitoring the ventilator, must observe: the type of ventilator; control modalities; the parameters of tidal volume and respiratory rate; oxygen fraction inspiration parameters; the inspiratory pressure reached and the pressure limit; the inspiration / expiration ratio; the volume per minute; sigh parameters, when applicable; the verification of the existence of water in the circuit and in the folds or the disconnection of the tracheas; humidification and temperature; alarms; and positive pressure levels at the end of exhalation and / or pressure support, when applicable. Therefore, the nursing professional acts fundamentally in ICUs; however, it is also observed that they, currently, in certain institutions, are becoming increasingly distant from ventilatory support, perhaps due to the numerous attributions that are deprived of them, or because there is another professional category doing this type of assistance, as well as for the deficiency of your knowledge Conclusion: as it is a delicate handling procedure, it is observed that the monitoring and full follow-up by the nursing professional is an extremely important factor to avoid severe and, sometimes, potentially fatal complications
